Individual diabetes self-management training

Baptist Health Louisville proudly offers a diabetes self-management training program recognized by the American Diabetes Association. This education recognition program signifies that Baptist Health Louisville meets or exceeds all national standards for diabetes self-management training, offering patients education to develop and improve self-management skills. These classes address the needs of people with both Type 1 and Type 2 diabetes.

Additionally, Baptist Health Louisville offers pre-diabetes classes in which participants learn how to make lifestyle changes to delay or prevent the onset of Type 2 diabetes. After completing the class, participants can choose to enroll in the NEED program (Nutrition, Exercise and Education to Deter Diabetes), developed as a collaboration between Baptist Health Louisville and Milestone Wellness Center. In this program, participants can work out in semi-private sessions with a personal trainer three times per week for four weeks or twice a week for five weeks. The goal of NEED is to delay or prevent the onset of Type 2 diabetes for those already diagnosed with metabolic syndrome which is characterized by extra weight around the waistline.

Group classes

National Diabetes Prevention Program (DPP) -- This program is offered periodically through a grant from the Centers for Disease Control (CDC) and the American Association of Diabetes Educators (AADE). A registered nurse and dietitian, who are diabetes educators, coach a group of 12-14 participants, focusing on lifestyle changes such as improving food choice and increasing physical activity) to prevent Type 2 diabetes. The program includes 16 weekly classroom sessions, plus six follow-up sessions scheduled throughout the year. Participants must meet specific criteria. Find out more.

Evening Group classes -- The classes usually begin the second Wednesday of the month (*unless otherwise noted) and are held for three consecutive Wednesdays from 6-9 p.m. Plan to come to class at 5:30 p.m. each week for paperwork. A registered nurse and dietitian present the classes. Blood glucose meters will be evaluated and an individualized meal plan will be provided. There is no extra charge for a guest. Individual diabetes self-management training

Diabetes individual educational session -- A registered nurse will work with you if you have newly diagnosed diabetes, previously diagnosed diabetes, gestational diabetes, diabetes management challenges, need insulin instruction or blood glucose monitoring needs, special needs, or topics as determined by you and your physician. Sessions are from one to two hours. There is no extra charge for a guest. For an appointment, call (502) 897-8831. Please print out and complete the individual patient information packet.
