Congestive Heart Failure Clinic
At the Congestive Heart Failure Clinic, Baptist Health Louisville brings together a highly-skilled team to examine your medical condition, assess your individual needs and formulate a plan to help you feel better. Your physician must refer you to the clinic.
The Clinic team includes a cardiologist, psychologist, nurse practitioner and dietitian. You and a family member or caregiver will meet with all of the team members at your initial appointment. Each will talk to you about your concerns and needs.
The team will create a treatment plan of medication, lifestyle and dietary changes that are customized for you. You, your family and caregiver(s) will actively participate in putting that plan into action.

A nutrition assessment is available here. You will need to complete it prior to your first visit. Be sure to bring it with you at that initial appointment.
If you are unsure whether you have congestive heart failure, often called CHF, check with your primary care physician. If you do not have a primary care physician, call Baptist Health Information Center at (502) 897-8131 for a referral.
Symptoms include fatigue, shortness of breath, wheezing or cough, fluid retention and weight gain, appetite loss and abnormally fast or slow heart beat.
